1) Whether this is an emergency 2) Your name (or the person’s name you’re trying to help) 3) Date of birth 4) Address 5) Brief explanation of the situation

After our conversation, we will e-mail you a .pdf with additional questions and materials to bring to our first meeting. If you can answer these questions before your meeting, it will save you from having to wait in the lobby scribbling out your responses on a clipboard.
